Heart Services 'You Said, We Did'

We held patient workshops to look at how we could make general service improvements to the way that existing heart services are provided. 

You told us: Our response: 

Access to services and GPs and continuity of GPs at practice level could be improved.

Mental health aspects are not addressed and could be better provided.

You think patients are often discharged into the community too quickly and require better access to service information upon discharge.

Communication needs to be improved with information shared between health professionals.

There was a lack in availability of rehabilitation and exercise options covering cardiology.

It was important to involve dietary information and discussion with experts on diet as part of care.

We will use this information to help shape service improvement and redesign, ensuring your expectations are listened to and embedded within transformation plans going forward.   We will provide further information as the service starts to transform.
